The BR 18.4 was built in the JA Maffei workshops in Munich. The first example was delivered on July 16, 1908. The locomotive's axles were of the "Pacific" type, with large-diameter wheels. It was the largest locomotive built by this manufacturer at that time. Today, it is considered "the most beautiful locomotive on the German railways."

The S 3/6 locomotive was highly efficient on long-distance journeys. In 1951, it hauled a 150-tonne train over 820 km between Hamburg and Munich, stopping only to take on water. The letter S stands for "Schnellzuglokomotiven" (high-speed trains), and the designation 3/6 indicates the ratio of driving axles to the total number of axles.

This locomotive was primarily used in Bavaria for long-distance trains, but also hauled such famous trains as the Orient Express and the Rheingold, the Ostend-Vienna Express, and the Paris-Karlsbad-Prague Express. Over time, this locomotive underwent numerous modifications, mainly affecting the driver's cab, the boiler, and the engine mechanisms.
